What Determines the Cost of a FHIR API Integration?

The work packages, hidden dependencies and commercial choices that affect FHIR integration scope and price.

The work packages, hidden dependencies and commercial choices that affect FHIR integration scope and price.

System and data complexity drive cost

A read-only API with clean source data differs substantially from a multi-system bidirectional exchange. Count the systems, resources, operations and partners involved—not merely the number of endpoints.

Implementation guides and test obligations

Scope changes with FHIR version, implementation guides, terminology, identity, consent and data-validation requirements. Clear acceptance criteria are a better price predictor than a generic “FHIR integration” label.

Production-readiness adds work

Include source-quality remediation, retries, versioning, logs, monitoring, security review, vendor coordination and handover. Treat migrations and legacy data as separate work packages where appropriate.

Choose the pricing model to match uncertainty

A discovery sprint can reduce uncertainty before a fixed-scope build. Where requirements will evolve, a time-and-materials allocation with milestone checkpoints may be clearer. Confirm which third-party licences and cloud costs are excluded.

What to include in your brief

Summarise source and destination systems, target resources, data volumes if known, access availability, security requirements, key deadlines and success criteria. A defensible estimate should list assumptions and exclusions.

ENGINEERING NEXT STEP

If you have a defined integration project, describe your systems and requirements at a high level. We can discuss whether a discovery sprint or technical engineering support is appropriate.

Tell us the systems, integration goal and delivery stage. We will discuss whether a bounded discovery sprint or engineering engagement is appropriate.
